Adult Case Management

Charlotte White Center, CWC, Community Case Management for AdultsThe Charlotte White Center (CWC) provides community case management services to adults with a diagnosis of intellectual or developmental disability. These services are intended to connect affected individuals with the skills and resources necessary for them to reduce or recover from any medical or behavioral symptoms that negatively impact the person’s quality of life. An additional service provided within case management is Representative Payee for assisting individuals with their social security income.

Team Planning

Utilizing our strengths based approach, including a focus on family, our staff works to create a partnership between the consumer and their family, identifying and prioritizing areas of concern. This newly formed team works to maximize its potential by determining the individual’s strengths and needs, and the presence or absence of skills and resources necessary for them to achieve their goals.

Our Case Managers then monitor the plan to ensure flexibility, so that it remains relevant to the individual’s specific needs as progress occurs and changes develop. This includes Representative Payee, if needed.

Strengths Based

CWC seeks to make the best use of the consumer’s natural support system by using the following strengths based approach:

  • The consumer chooses his/her support team
  • Planning is customized to his/her needs
  • The team process involves discovering and using the skills and resources available to him/her. These identified “strengths” are used to address his/her overall needs.
  • The consumer receives ongoing support from our community integration staff and other providers as needed.

Referral Process

Once an individual has been found eligible for adult Developmental Services and has chosen CWC to provide their Case Management, a member of the Case Management staff will begin the intake process. A CWC staff member will contact you and schedule an appointment for initial intake.

Areas of Service

At present our service network extends to Piscataquis, Penobscot, Hancock, Somerset, Waldo and Kennebec Counties.

Eligibility:

  • Each individual must have been determined eligible for DHHS/Adult Developmental Services by DHHS / Developmental Services Regional Supervisor
  • Must be 18 years old or older
  • The individual cannot be under public guardianship
  • Must have MaineCare or be eligible for it
  • The individual must not be receiving community, home or work supports from CWC
