In-Home Children's Supports (Sec. 28)

Charlotte White Center, CWC, Section 28, In Home Children's SupportCWC’s Rehabilitative and Community Support Services for children are one-on-one services provided to a child/adolescent in his/her home or community setting. The focus is primarily on behavior management, skill acquisition and physical development activities. The goal of these services is to achieve an increase in a child’s level of functioning, an increase in skills development and a decrease in maladaptive behaviors. CWC requires the active involvement of the consumer’s family, parent/ guardian and the family’s natural supports in this process.

These services are provided under the direction of a professional staff person with the skills, training, education and experience sufficient to meet the expectations of DHHS’ high standards. Support staff will meet the qualifications required for the Behavioral Health Professional position.

Services Provided:

The team works with the child and family and other providers to:

  • Collaborate with the child’s family, school, therapists, case managers and any other key individuals to identify strengths and needs.
  • Develop a comprehensive psychosocial assessment.
  • Facilitate the individual program planning process.
  • Develop an Individual Service Plan for each child with a minimum of quarterly reviews.
  • Match and assign a Behavioral Health Professional to work with each child in the least restrictive setting available.
  • Design programs to match the health, safety, and human rights requirements of the state of Maine and Federal statutes.

Service Mix:

Rehabilitative Services identified in the child’s treatment plan are provided to the child by his/her Behavioral Health Professional in the child’s home or a community setting, and include but are not limited to:

  • Daily living skills
  • Behavior management
  • Coping skills
  • Time management
  • Appropriate social interaction skills
  • Community integration skills
  • Increasing self-esteem/self-confidence
  • Following directions and staying on task
  • Developing and maintaining the child’s sensory, motor, and psychological skills development
  • Physical development activities to promote selfmaintenance, self-awareness and self-motivation

Eligibility

Children and adolescents qualify as follows:

  • Child is birth to age twenty.
  • The child/adolescent must have a mental health diagnosis, cognitive impairment, or functional limitations in significant areas of development.

Children will meet the above criteria and are in need of services due to:

  • The child’s behaviors
  • Physical, sensory, motor, or psychological impairments
  • The child’s needs cannot be managed solely by the parent/guardian

and

  • The child is within the area of CWC coverage.
  • CWC has the appropriate trained staff available for the primary diagnosis.
  • The child is eligible for or is covered by MaineCare.
