Supportive Living Program

Charlotte White Center, CWC, Supportive LivingThe Charlotte White Center’s Supportive Living Services for Adults with Intellectual and Developmental Disabilities provide an individual with a comfortable home shared with one other resident, and 24/7 trained staff. The assigned staff members assist the residents in attaining skills to help them to be as independent as possible. Each resident has his/her own bedroom and receives individualized care tailored to his/her own treatment plan.

Eligibility:

An individual is eligible for this service if he/she meets the following criteria:

  • Must be 18 years of age or older
  • Must have been diagnosed with an intellectual or developmental or disability
  • Must have DHHS funding (MaineCare Home and Community Based Waiver Approval-Section 21)

Supportive Living Services Provided

Charlotte White Center’s Staff provide residents with daily assistance in meeting the needs identified in their individualized treatment plan, which may consist of assistance with:

  • Choice making and independent living skills training
  • Community integration and inclusion
  • Developing socially appropriate behavior and interpersonal skills; including anger management, increasing self-esteem and coping skills
  • Developing and maintaining age appropriate behaviors; including socially appropriate behavior and interpersonal skills
  • Developing functional skills; including, but not limited to, properly handling emergencies, cleaning and maintaining their home, managing money, recognizing street signs, problem solving skills, etc.

Reliable Staff

Our staff administers and monitors all medications, as well as reports any reactions to the resident’s doctors, when this is a need.

Our staff also refers residents to other appropriate services, such as counseling, occupational and physical therapy. Our staff schedules medical and dental appointments, as well as coordinates transportation to and from.

Goals and Objectives of Supported Living Services:

  • Maximize independence and provide the least restrictive environment possible while ensuring safety.
  • Create an environment of choice for the individual whenever possible.
  • Ensure that the health, safety, and well-being of the individual is preserved and respected.
